For all new patients, please download & complete the patient registration form below ahead of your visit.


We understand that occasionally you may need to cancel an appointment. If that happens, we respectfully ask for scheduled appointments to be cancelled at least 2 hours in advance.

There is a fee of $20.00 AUD for appointments that are cancelled within 2 hours, if you have a late arrival, or you do not attend your appointment.

This fee needs to be paid prior to booking your next appointment & is not claimable with Medicare or health fund.

This policy will enable us to open otherwise unused appointments to better serve the needs of all patients. If someone has missed 3 or more appointments in a 12 month time period then their name will be removed from the practice patient list.


Email Policy :
Please note email requests of referrals/scripts incur a minimum private fee of 40 AUD. Please contact the practice reception team to pay the fees first if you want to utilise this service. If you want a referral/script on an urgent basis then the minimum fee of 50 AUD will be charged. Please contact the practice reception team to pay the fees first if you want to utilise this service.

Zero Tolerance Policy :

We have a Zero tolerance approach to any rude, verbal, aggressive and violent abuse and behaviour towards our staff or other patients. We respectfully advise you that abuse and/or violence will not be tolerated and may result in immediate removal from the practice list.


Practice Policies:


Appointment Policy  :

Mymedical health centre only works on a booked appointment basis. Please contact the practice or book online via our website or HotDoc website/app to make an appointment prior to your arrival. We do not accept Walk-in patients.

New patient policy  : 

All new patients must arrive 10 minutes before their appointment time. This is to ensure the nurse will complete the necessary health check before you see the GP. This is to avoid any issues in the future and ensure a high quality of care for every visit. As per practice policy new patients will not be seen by a doctor if they fail to come 10 minutes before their appointment time.

Late arrival :

We only work on a booked appointment basis therefore it is important that all patients should arrive on time. If someone is more than 10 minutes late, they will not be seen and cancellation charges will be applicable on late arrival. The patient will then have to book an appointment at another time.

Consultation Fees :

We provide bulk billing for most Consultations with the exception of new patient telephone consultations as per Medicare billing policies  ( private fees apply). If someone requires a prescription or referral without a telephone or face to face consultation then 40 AUD will be applicable.

Gap Fees : 

We provide bulk billing for most consultations however Gap fee applies to some procedures ( Implant insertion /removal, Coil removal, Iron infusion, Dressing, Cryotherapy, Skin Check, PRP Joint injection, Dietician, Physiotherapy and Podiatrist appointments ). Please contact reception if you require further information. There is no gap fee for patients less than 16 years and 75 years and above in age.

Saturday Consultation :

We also bulk bill for Saturday consultations with the exception of new patients. 25 AUD gap fee will be applied to all new patients seen on Saturday. The Saturday appointment duration is 10 minutes therefore if you are more than 5 minutes late on Saturday then you will not be seen and a cancellation fee of 20 AUD will be applicable on late arrival.


Telephone consultations : 

Telephone consultations are available on a daily basis. If you are unable to book a face to face appointment, have mild covid-19 symptoms and want to discuss blood test results/script/referral or any emergency issues then please contact our practice to book telephone consultations. We bulk bill most telephone consultations with the exception of new patients.
