patient registration form below ahead of your visit.

CANCELLATION POLICY :

We understand that occasionally you may need to cancel an appointment. If that happens, we respectfully ask for scheduled appointments to be cancelled at least 2 hours in advance.

There is a fee of $40.00 AUD for face-to-face and 20 AUD for telephone appointments that are missed or cancelled within 2 hours, if you have a late arrival, or you do not attend your appointment. An $80 cancellation fee applies if a long appointment is missed. 

This fee needs to be paid prior to booking your next appointment & is not claimable with Medicare or health fund.

This policy will enable us to open otherwise unused appointments to better serve the needs of all patients. If someone has missed 3 or more appointments in a 12-month time period then their name will be removed from the practice patient list.

 

Email Policy :
We do not accept any email requests for referrals and scripts.

Zero Tolerance Policy :

We have a Zero tolerance approach to any rude, verbal, aggressive and violent abuse and behaviour towards our staff or other patients. We respectfully advise you that abuse and/or violence will not be tolerated and may result in immediate removal from the practice list.


 

Practice Policies:

 

Appointment Policy  :

Mymedical health centre only works on a booked appointment basis. Please contact the practice or book online via our website or HotDoc website/app to make an appointment prior to your arrival. We do not accept Walk-in patients.

New patient policy  : 

All new patients must arrive 10 minutes before their appointment time. This is to ensure the nurse will complete the necessary health check before you see the GP. This is to avoid any issues in the future and ensure a high quality of care for every visit. As per practice policy new patients will not be seen by a doctor if they fail to come 10 minutes before their appointment time.

Late arrival :

We only work on a booked appointment basis therefore it is important that all patients should arrive on time. If someone is more than 10 minutes late, they will not be seen, and cancellation charges will be applicable on late arrival. The patient will then have to book an appointment at another time.

Consultation Fees :


Starting 1/4/24 $50 gap fees will be applicable to all patients except children aged 10 & under & DVA card holders (weekdays only). Pension, concession, health cards and children aged 10 to 14 require a mandatory MyMedicare enrolment with the practice for bulk billing from Monday to Friday. Private fees apply otherwise. There is a 20% discount on private face to face appointment fees with Mymedicare enrolment.
   
Gap Fees : 

Gap fee applies to all appointments including services such as implant insertion /removal, Coil removal, Iron infusion, Dressing, Cryotherapy, Skin Check, PRP Joint injection, Dietician, Physiotherapy and Podiatrist appointments. Please contact reception if you require further information.

Saturday Consultations :

$50 consultation fees apply to all ages and all types of appointments on Saturday. There is a 20% discount on private face to face appointment fees with Mymedicare enrolment. DNA Fees will be applied (Please check full policy). (outstanding fees need to be paid before the appointment ).


Telephone consultations : 


Due to changes to Medicare billing consent requirements, we will unfortunately no longer be able to offer any bulk billed telehealth consultations. As a result, all telehealth consultations will be charged at $60 for weekday appointments and $85 on a weekend. These charges will apply regardless of length of consult, with a maximum of 10 minutes allowed for telehealth consults. Payment will be collected prior to your appointment, and you will be emailed an invoice for your appointment. This will then enable patients to claim the Medicare rebate for their appointment. This rebate will vary depending on various factors within the consultation.
